A modern sax just fits length wise and thickness wise. The foam takes a while to adjust to a custom fit. The accessory area is tiny. It just fits some reeds, a spare mouthpiece, swab, lip balm, cork grease, etc. A neckstrap will not fit, though, so it must go down the bell. If the mouthpiece and neck had vertical storage spots on the opposite side, the accessory area could be vastly increased and could then easily hold a neckstrap and much more. The case is fairly light and the handles appear to be well connected to it. Haven't tried the backpack mode, so can't comment. The outside pocket has plenty of space for sheet music. There is only one small pocket on the outside of the main pocket though it looks like there should be two.
